I cannot find my birth certificate. What can I do?
If a birth certificate cannot be found, you MUST have an affidavit from the Custodian of Records in the county or state of your birth stating that a birth certificate is not on record. In addition, at least three documents MUST BE submitted to substantiate the date and place of your birth.

Some examples of acceptable documents are:
  • Certified copy of school record
  • Old family Bible
  • Baptismal record
  • Old insurance policy
  • Affidavit of physician, midwife or person older than the applicant who has definite knowledge of the birth.
  • Copy of Federal Census Record
  • Social Security application or any other convincing evidence
How do I get a certified copy of my birth certificate from another state?
You may contact the Passport Clerk for the address and telephone number of the Office of Vital Statistics for your birth state.
Can I use home snapshots for my passport photos?
No, your passport photos must be two "identical" photos, frontal head shots, 2 inches by 2 inches taken by an authorized passport photographer.
How long will it take to obtain my passport?
Allow six weeks for passport processing. Currently it is taking 20-30 business days (4-6 weeks) from receipt of completed application by the Passport Agency.
I am leaving the country in 4 weeks. Can I get a rush passport?
Yes, customers wishing to receive passports sooner may request Expedited Service. You can receive passports in 2-3 weeks for an additional fee of $60.00 per application and overnight express mail postage.
How long is my passport good for?
For adults and minors age 16 and over , your passport is valid for 10 years from the date of issuance. Passports of children 15 and under are valid for 5 years from the date of issuance.
What about a visa?
A passport must be obtained before the government of a foreign country will issue a visa. Travelers should contact the consular officials of the countries to be visited well in advance of their departure. It is the responsibility of the applicant to obtain his or her own visa.
How do I renew my passport?
If you wish to renew your passport and it was issued less than fifteen (15) years ago, ask the Passport Clerk for a DS-82 Form. It is your responsibility to complete the form in black ink. The instructions for completion and mailing are located on the back of the form.
If I have a problem after I submit my application, who do I contact?
The Clerk's Office acts as agent for the Miami Passport Agency. The Agency makes all final decisions regarding the passport applications and/or documentation.
